Hi all,

Here is an interesting case I wanted to post in the discussion group:

We saw a patient at  12 weeks with increased NT of 3.3 mm. Other relevant images enclosed. What

would you think in your differential diagnosis?

My differential diadnosis:

1.T 21

2. T18

3.Major Cardiac Anomaly

 
This wasa trisomy 13 case. the heart was on the right side of the chest, and there was mild tachycardia to 190-200 bpm ranges. Apparently ay 10-14 weeks, fetal heart rate can help fro sorting out the aneuploidy: In trisomy 21, trisomy 13 and Turner syndrome fetal heart rate is high and  in trisomy 18 and triploidy the heart rate is low. In 67% of t13 fetuses, the fetal heart rate is above the 97th P .

Hi Dave,

 

The first image shows the heart and the stomach on different sides on a coronal image. Second one show the fetal heart rate in the 188 bpm. I think the last one was not the best one to choose from, it is a transverse-oblique that shows the heart and the stomach on different sides again. I enclosed a new image with labels. The fetus had an increased NT, diaphragm appeared intact, and nasal bone was present.
