A lady with history of urinary incontinance & vaginal discharge sent for pelvic Us . On examination an irregular echogenic mass found within bladder. The mass gives posterior shadowing but does not moves with posture change. She had H/o vaginal hysterectomy 9 month back.
In the light of history with Us findings the case was diagnosed as Gosiphiboma ( retained surgical cotton piece ) which confirmed after laparatomy.
Now the Big ? how it migrate to the bladder.
The Gynecologist ( who performed both operation ) explain that the cotton piece soked with lignocane used for local aneastetic purpose at vaginal vault & left it by fogetfulness. He doesn't injury to bladder.

The posibility exist of portion of the packing migrated to the uretra iand in to the bladder in this patient with bladder wall weakening.
Thanks for reply & valuable comment. The cotton piece found within bladder adherent with it's wall. Upon removal there was some frank pus in a pocket at bed which was not identified during sonography. There may have weekness at bladder wall as you rightly mention it. But migration trough urethra was not as the cotton piece was the size of a rosted chiken thigh.
