its probably best to just send this directly to the home office. there are several downloads. the process is described in detail.

First, you'll want to know if you and any other interpreting physicians in your practice meet the AIUM training guideline in MSK ultrasound (http://www.aium.org/publications/viewStatement.aspx?id=41).
Next, you'll want to be sure that the MSK exams you perform meet the MSK performance guideline (http://www.aium.org/publications/guidelines/musculoskeletal.pdf). Please note that not every practice will perform every exam listed in the guideline.
You can access our accreditation standards and guidelines at http://www.aium.org/publications/viewStatement.aspx?id=26. There are also two webinars on applying for AIUM accreditation. The first goes over the online portion of the application and supporting documents, and the second reviews case study submission requirements. These webinars are available to everyone at no charge (AIUM members may take the associated CME tests and earn free CMEs).
