Does anyone have any examples of 'pink appendicitis' on ultrasound? These are viral appendicitis cases where the wall is thickened but the appendix isn't fluid filled and distended like in acute suppurative appendicitis. How do you deal with those cases?

Sorry for joining the pediatric forum late. I have heard of CATARRAH (FLU LIKE) appendicitis. In my opinion, what matters is the clinical picture: ? tender ? fever ? nausea/vomiting. If the patient is stable, conservative Rx and careful clinical ans sonographic follow up is all that is required.

There was an interesting presentation on this at SPR a few years ago. Basically, children present with signs of appendicitis but pathological examination reveals nothing except lymphoid hyperplasia of the wall and adenovirus infection. The thought is that the appendix is reacting as part of a generalized viral infection (in the same way the mesenteric nodes do). The presentation had some nice examples of slightly thick walled appendixes but nothing like the acute suppurative appearance. Does anyone remember this presentation? The appendix is slightly pink on pathological examination. Ultrasound is good, but it can't pick up the pink color!
