Proper Ultrasound Coding?

What are other outpatient ultrasound facilities in the US coding when an abdomen complete (76700) and a inguinal hernia (76705)ultrasound is ordered on the same patient at the same time? What about a pelvic ultrasound (76857/76830) along with a inguinal hernia? Abdomen complete and Renal/Bladder (76770)?See More

Brenna Crain replied to Jessica J Budd, RDMS's discussion ureteral calculi in the group General & Abdominal Ultrasound Community
"I have seen it quite a few times, you can even see the ureter peristalsing around the stone. Matter of fact it is a pretty decent way to interrogate the trigone area where the ureters insert if you cannot get close enough transabdominally."

"We often see this phenomenon. We actually refer to the cervix as a dynamic structure so when we do endovaginal exams we usually watch it for a minimum of 3 minutes or so to watch for these changes."

Bill Goodnight replied to Brenna Crain's discussion Coding 76811 vs. 76805 in the group Obstetric Ultrasound Community
"The Society for Maternal Fetal Medicine has written suggestions for how 76811 should be used (see SMFM web site www.smfm.org). The main points that differentiate 76811 from 76805 include (from SMFM white paper): a) CPT 76811 is not intended to be…"

"This is great information outlining the difference between codes. Your information states..."CPT 76811 is not intended to be the routine scan performed for all pregnancies." Is it not the standard of care in most dedicated outpatient…"

"Laura... We have currently been in a major discussion about this recently in our facility, in fact this is the main reason I joined this group. In your case I think Aetna is right. At our facility in WA state, we have always performed ALL of the…"

"For routine second trimester scans, we code 76805. I feel it is the responsivbility of the sonographer to assess all fetal anatomy during every second trimester scan. The code of 76811 is used when there are cases of AMA etc. We also look at…"

Coding 76811 vs. 76805

For routine second trimester anatomy scans, are you coding a 76811 or a 76805? According to the ACR the 76811 is only for "high-risk OB's" but when it lists all of the views it requires it is what we do for ALL of our anatomy scans.  We look at everything except perhaps counting fingers and toes.  What is the standard out there for routine second trimester scans? Also, do you code for 76820 (umbilical Doppler) as well as 76821 (MCA Doppler) when done? See More
