Peter J. Mariani MD replied to Jason T Nomura MD, RDMS's discussion An article entitled, "The Trouble with Ultrasound’s Pervasive Use by Non-Radiologists". Really? in the group Emergency & Critical Care Ultrasound Community
"There's been a lot of dyspepsia posted today to the ACEP Ultrasound Section listserv in reaction to this article. I think it's an over-reaction. The article and its contents have not been endorsed by AIUM; the quoted physicians are…"

Peter J. Mariani MD replied to Robin Hurni's discussion Interventional pain mangement in the group Emergency & Critical Care Ultrasound Community
"A fair amount of virtual ink has been recently given this topic on the ACEP [American College of Emergency Physicians] ultrasound interest group listserv. Ownership of the machine is one variable determining whether a given entity can bill the…"

Peter J. Mariani MD replied to Pattie Smith, RDMS's discussion Hand held ultrasound units in the group Emergency & Critical Care Ultrasound Community
"For my purposes of ED or hospice home visits, current standard portable machines are plenty small enough for me. If I were trekking in the 3rd world bush or perhaps passing an instrument down a test shaft to Chilean miners, the philosophy might be…"

"We currently use Sonosites (Titan and MTurbo) as standard clinical purpose units. The ultra-small prototype was eval'd under proprietary agreement with its manufacturer. --PM"

Peter J. Mariani MD replied to Robin Hurni's discussion Interventional pain mangement in the group Emergency & Critical Care Ultrasound Community
""Can a doc start a pain clinic..." is a legal question impacted mostly by the law of the individual state of the doc's licensure. Most states are rather laissez faire once a doc attains minimal license muster. It could also be…"

Peter J. Mariani MD replied to Pattie Smith, RDMS's discussion Hand held ultrasound units in the group Emergency & Critical Care Ultrasound Community
"Our dept is currently trialing/beta-testing one manufacturer's prototype. Truly hand-held units --as opposed to those only hand-carried ("HCUs")-- will eventually become mainstream. What'll limit the ongoing shrink won't be…"

Jason T Nomura MD, RDMS replied to Peter J. Mariani MD's discussion Additional CVC Sonographic Confirmation Technique in the group Emergency & Critical Care Ultrasound Community
"it is probably possible to gauge the distance from bubble injection to appearance in the right atrium. But the timing form injection to differentiate between subclavian and IJ is probablyso slight that I think you will need a computer analysis to…"

Peter J. Mariani MD replied to Peter J. Mariani MD's discussion Additional CVC Sonographic Confirmation Technique in the group Emergency & Critical Care Ultrasound Community
"Jason-- Thanks for the good background info. I'm wondering whether a mechanical pressure injector --as you mention-- might permit study of the kinetics of bubble appearance. Are differences in time to bubble appearance (or to maximum…"

Jason T Nomura MD, RDMS replied to Peter J. Mariani MD's discussion Additional CVC Sonographic Confirmation Technique in the group Emergency & Critical Care Ultrasound Community
"I like this method and have used it many times. Usually when I use this is when I am called into a room for a line that doesn't seem right that was placed by someone when I did not see the US images of placement. You don't need to agitate…"

Additional CVC Sonographic Confirmation Technique

In this month's Academic Emergency Medicine (electronic version), EPs from Hennepin describe an ostensibly appealing method of line location confirmation using 'deliberate but not deliberately deliberate' microbubble contrast echography. Evidently, garden variety saline --as is-- if centrally injected has inherently sufficient microbubbles to clearly manifest on echo. See attached .mov made contemporaneously with injection. I had no idea, and am anxious to try. Has anyone else had experience…See More

Peter J. Mariani MD replied to Jason T Nomura MD, RDMS's discussion Ultrasound for Confirmation of Central Venous Catheter Position in the group Emergency & Critical Care Ultrasound Community
"I personally use the technique, but haven't had "the spine" to thereafter omit the CXR. One could theoretically cause a PTX, get back intraluminally, and then "successfully" insert the cannula. Insonating the vein would now…"
